The GH/UW subjects produced a total of 300 cases and 4,475 controls.  For cases, 64, 47 113 and 76 subjects qualified by algorithm rules 7.2, 7.3, 7.4 and 7.5, respectively (see Pseudo Code document for details).  For controls, 191 subjects qualified by rule 8.1 and 4,284 qualified by rule 8.2.

For validation we manuall reviewd the charts of 25 cases selected at random and 25 controls selected at random.
